Submit Your Patient Information via EasyRx^®

BEFORE YOU BEGIN
Park Dental Aligners uses EasyRx^® to manage our lab prescription workflow. Before you submit you patient case information, you will need access to the EasyRx^® platform. If you are an existing EasyRx^® member, simply follow the instructions below and submit your patient information to our Park Dental Aligners EasyRx^® digital lab. If you are not a member of EasyRx^® please contact us at Support@ParkDentalAligners.com or call our offices at 1-800-243-7372 and you will be provided with an invitation to access the EasyRx^® platform as a guest, at no charge. Once you have been granted access simply follow the steps below.

Submit Patient Information:
1. After ordering our Park Dental Aligner Impression Kit, take your patients impressions (or use your own impression material and make a stone model)
2. Take patient bite photos (with oral cheek retractor, left, right and center)
3. Mail us your patient's impressions or stone models. Use the prepaid return label in the Park Dental Aligner Impression Kit and send via FedEx or UPS to:

After impressions/models are received and scanned, the STL file will be uploaded to EasyRx^®

*If you have an intra-oral scanner, you can scan your patient and submit their bite photos along with the digital scans (.STL file) to Park Dental Aligner's EasyRx^® Digital Lab.
